It's a photo from the June 17, 1916 issue of Moving Picture World. The image was on Flickr in the collection of Charmaine Zoe. Thanks to Jack Tillmany for finding the article it came from and adding more data.
He notes that its first appearance is in the phone book published June 1, 1910. The last appearances are in the phone book of February 1, 1917 and the city directory of June 1917. The location of the now-vanished building was the southeast corner of Polk and Washington.
Seating: 400
Status: Demolished
