The Gem Theatre

 Opened: June 16, 1909. It was on the east side of the street between Pacific and Broadway. When the theatre was operating, the street was called Montgomery Ave. It was later renamed.
 

Thanks to Jack Tillmany for finding this item in the Saturday June 26 issue of the newspaper L'Italia that announced the opening.

Closing: Jack notes that July 1909 seems to be the end of it. The site was taken over by the Golden West Bakery who filed for bankruptcy in October 1911. 

Status: The building on the site dates from 1909. But it's unknown whether it was constructed before or after the theatre's brief life at the location.
