Jack Tillmany's 2005 Arcadia Publishing book "Theatres of San Francisco" can be previewed on Google Books. It's available from Amazon or your local bookseller.
The publisher wasn't interested in including an index. But one of the readers, Roy E. Pipher of Stroudsberg, PA, made one for himself in 2009 and sent it off to the publisher. Arcadia forwarded a copy to Jack and now it's reproduced here.
